After you schedule, you will receive forms to fill out that will give us a better understanding of your medical background, overall health, and weight loss goals. This will help us to determine the most appropriate treatment during your consultation.

We recommend our Membership Package for white glove service throughout your weight loss journey. We can prescribe any necessary lab tests and medications as appropriate.
